Financial & self-pay information
Patients must provide complete and accurate insurance information and choose how each visit will be handled before care is provided. Insurance on file does not by itself authorize Cutis MD to submit a claim for a particular visit.
- Insurance billing: coverage and network participation vary by plan and product. Copayments, deductibles, coinsurance, and non-covered amounts are determined by the health plan’s Explanation of Benefits.
- Direct pay: $275 for a new-patient medical visit and $195 for an established-patient follow-up. Procedures, medications, laboratory testing, and pathology are separate.
- Virtual visits: $150 direct-pay fixed rate for an appropriate focused concern or follow-up while the patient is physically located in Georgia. If an in-person visit is needed, it is scheduled and priced separately.
Commercially insured patients may request direct pay only when permitted by applicable requirements and Cutis MD’s payer agreements. Direct-pay visits generally are not submitted to insurance and generally do not count toward a deductible or out-of-pocket maximum. Medicare, Medicaid, TRICARE, and other government programs may have different requirements.
Deposits, cancellations & balances
A $25 deposit may be required to secure an appointment and is applied according to the policy shown during booking. Patients should review the booking confirmation for the applicable cancellation or rescheduling window. Late cancellations and missed appointments may result in loss of the deposit or another disclosed fee. Outstanding balances or payment arrangements may need to be addressed before additional non-urgent services are scheduled.
Good Faith Estimates
If you are uninsured or choose not to use insurance, you may have the right to receive a Good Faith Estimate of expected charges under federal law. Ask Cutis MD for an estimate before scheduling or receiving care. The estimate may not include separately billed services from outside laboratories, pathology groups, pharmacies, imaging facilities, or other clinicians.
Website privacy & communications
Information submitted through appointment requests, contact forms, email, or text may be used to respond to you, coordinate care, and operate the practice. Standard email and text messaging may not be secure. Please do not send photographs, detailed medical histories, or other sensitive health information by ordinary text or email unless Cutis MD specifically directs you to an approved secure method.
Our website platform and service providers may process technical information such as device, browser, and usage data to operate and improve the site. This website privacy summary is not a substitute for Cutis MD’s formal Notice of Privacy Practices, which is available from the practice.
